FSA Root Cause Analysis Project

The FSA has commissioned a project to review its dissemination of root cause analysis information.  This information is routinely requested following an incident but there has been limited sharing of the findings.  

The project intends to assist FSA in determining the best way to deliver this information  to provide benefit for industry and reduce the occurrence of incidents.  The project report and links to further information are available by signing in.
